Please remember, we require a 48 hour notification for all appointments. Any appointment cancelled within the 48 hour window prior to a scheduled visit will be charged $20 non refundable, cancellation fee. Not having the blood tests performed in time to review at the visit is not a reason for exemption from this $20 cancellation fee.
The Fee for Service Rate in 2025, is $150 for Return Visits.
